    Cabinet for Health Services, Office of Vital Statistics, 275 East Main Street, Frankfort, KY 40621
    This office maintains birth and death records from January 1911; and marriage and divorce records from June 1958. Records prior to these years are maintained in the county clerks office. To request a certified copy of an official record, you may print out and mail or fax the appropriate application form. Of course you can always order in person or by phone. Our office takes orders over the phone from 8:00 a.m. to 3:00 p.m. The customer has to have a valid Mastercard, Visa, Discover or American Express. The fee is the required fee for the vital record plus an additional $5.00. This request will be mailed within three(3) business days. If the customer wants next day delivery by Federal Express the fee is the cost of the vital record, plus $5.00 handling plus $10.70 for Federal Express.
